The Initial Consultation

In our initial consultation, we’ll sit down to talk about your situation and goals. From there we’ll create a personalized plan of action that may include any of the services listed below. Once the plan is in motion, regular home, hospital, or facility visits can be scheduled to monitor and optimize your loved one’s care.

Care Coordination

We are like a project manager for your loved one’s health. We’ll connect you to the appropriate medical and non-medical resources, schedule appointments, follow up after appointments, and make sure everyone on your health care team is working together to achieve the same goals. We’ll even help you manage any insurance issues.

Patient Advocacy

We are your voice in the healthcare system. We can accompany you to appointments, and talk directly to the physicians, medical service providers—even insurers—to address your concerns, translate “doctor-speak,” and advocate on your behalf for the most appropriate care.

Disease Treatment & Disability Care Planning

With the expertise that only a registered nurse can provide, we will help you understand and evaluate your loved one’s treatment options and plan for any necessary disability care. We aim to treat the whole patient—physically, emotionally, mentally—for not just better health, but better quality of life.

Resources & Referrals

Oftentimes, patients and families don’t know of all the support services available to them. We do. We’ll show you ALL your options—including an evaluation of quality and cost—to make sure you’re optimizing every resource at your disposal. We can also help obtain referrals when needed.

Continuation of Care Planning

It’s never easy to decide the next step for an aging loved one. But you can be educated. We’ll provide a clear, unbiased analysis of your options so that you can make a completely informed decision.

Independent Living Planning

If your loved one is independent, we can connect you to the medical and community services that can ensure his or her physical, psychosocial and legal needs are met. We’ll even assess the home and provide recommendations on simple modifications you can make to ensure daily activities, such as bathing, cooking, and stair climbing, can be done safely.

Long-Distance Family Liaison

For families who live away from their loved ones, we can be your bridge to the local healthcare system, helping you get the best care for your family member and keeping you involved, even across the miles.

Crisis Care Management

In the event of a crisis, we are available immediately to help. When necessary, we can file for court hearings to obtain emergency evaluations and to achieve immediate placement in an appropriate medical facility.

Flat-rate initial consultation fee.

Hourly, weekly or monthly consulting and care service fees determined individually.
